JOB TITLE: Travel/Contract Nurse - LPN
LOCATION: Valdosta, US-GA

HOURLY RATE: $15 - $18

DESCRIPTION:
JOB TITLE: Licensed Practice Nurse
JOB TYPE: Contract
JOB DURATION: August 1 - September 30, 2004; Two (2) 1 year options for
renewal.
JOB SETTING: Hospital - Ambulatory Care Unit; Monday - Friday 7:30 AM - 4:30
PM (No Holidays).
JOB DESCRIPTION: Duties: (1) Coordinate and assist in providing educational
programs for beneficiaries and staff to facilitate prevention and management of
healthcare problems (2) Alternate for oversight, guidance training to PCM
(Primary Care Manager) teams in the use of the Preventative Health
Assessment/ Individual Medical Readiness program. (3) Serve as resource to
patients/ significant others as related to health education (4) Maintain
current resource materials needed for patient education (5) Collaborate with
Physicians and other professionals in developing teaching materials
(6) Lead and participate in interdisciplinary patient education meetings (7)
Determine patients needs both as individual and as a population group for
education or care planning (8) Provide direct patient care in support of
providers within the Medical Treatment Facility (MTF)
JOB QUALIFICATIONS: (1) LPN license in GA (2) 2 years experience (3) BLS
(ACLS is desired but not required) (4) DEA Certificate of Applicable
(5) Professional Resume with sworn affidavit of truthfulness of same,
indicating experience, training, and technical expertise in the type of care
rendered.
(6) List of states RN holds a license (7) Physical Exam stating the employee
is free of any contagious disease (done within last 12 months)
(8) Immunizations: Hepatitis B, MMR, TB
