Incipient caries generally get either resin or amalgam depending on what the
patient wants. I offer gold, amalgam, resin and ceramic and let the patient decide. Most often they choose amalgam for price, and resin second. Some will choose ceramic for these. I am typically doing Cerec restorations on those teeth which we would normally place a core and a crown. I adjust the proximal contact with a HP shank diamond disk. Once the flossing contact is perfect, I treat the porcelain and bond it to place. Once bonded, I adjust the occlusion and polish. I prefer to polish in the mouth so that the margin will become imperceptible no matter where I place it. With the 3D software you can control proximal and occlusal contacts on the screen just as if you were sitting at the bench with articulated models (except excursive motion checks). You can "wax" it to perfect contacts and just bond it to place. Often however, once magnified, you will find slight scratches on the proximal tooth, over-contoured restorations next door, or tipped teeth. So you build out the proximal contact on the screen and adjust everything in the mouth to fit the way you want it to be. Ask WB how much adjustment we needed on those crown we did for Andrew. Yesterday, a relative was here from out of State. Needed RCT on a molar. We did RCT and crown with attached post in a bit over two hours complete. On a plane flying home this morning. It took a little longer since I was having fun teasing and joking with the patient and my DA. I had to touch the proximal contacts once with the disk to get perfect flossing pressure, and touched the occlusion three times with a diamond to get the occlusion I desired. No the material is definitely NOT the same. Cerec materials are all
pre-fired in a factory under identical conditions to achieve very specific material characteristics.
