Sure,

Carbocaine WITHOUT epinephrine will do the trick nicely. Apicoectomy
means the tooth is already devital so profound anesthesia is not a
problem.

Besides a nerve block I would consider local infiltration and it will
do the trick nicely!

Why???

I like blood actually ... it promotes healing (in healthy
individuals).

Beware of non-bleeding extractions ----> dry sockets!

Something tells me there is a misunderstanding here.
There are good anesthetics with no vasoconstrictor
(Mepivicaine 4% Plain comes to mind)
and for long term anesthesia with a small amount of
epi, Marcaine 1/200,000 epi works quite well.

The epi can be kept to a bare minimum and is not
actually required at all IMO.

It's a good idea to have profound anesthesia when doing an apico.
The tooth may not have feeling but the bone sure does.


There are other thing that can be used.
Bone wax, a non epi hemostatic agent, etc...

True, however a nerve block usually works for that. Patients have
always mentioned that getting a tooth yanked is far less traumatic
than getting one filled!

Mepivicaine (brand name Carbocaine) plain is in 3% Citanest
(prilocaine) plain is 4%
