Default FAO Alan Hardy (lantus down)

Hi Alan,
First of all can I apologise for contacting you personally this way.
The reason is that my email server will not send my emails and I would
like a speedy answer , if you can, that is .
Firstly , regarding my recent problem of erratic bgs and my Increased
Lantus dose and smaller novorapid doses consequently which woked very
well but for the morning lows.
About 10 days ago , I did as you suggested and kept my basal to 24
units and my boluses to each. This generally went well, except that i
went down to 3 and a bit on three fbs and one morning this week I woke
up at 2 a.m. with a very bad hypo. I only just made it - must have
been below 2 I reckon Again yesterday a.m. I woke at 3.1 bg after
dinner and late evening bgs were good and normal evening snack.
So last night I took 1 u less of lantus and this morning tested at 7
o'clock with a 12.3 bg!! I had normal b/fast , took an extra 3 u of
Novorapid , walked 2 miles briskly, and two hours later my bg was (
wait for it) 17.3 !!!!!! Could not believe it, so I re-tested and
was 17.6. Went down to 12.2 bef/lunch , took and extra 3 u of Novo
,tested two hours later and was 6.2

1.Can you think of a reason for this huge spike this morning?

2. I shall be staying with 23 u of Lantus tonight to give it a fair
trial, but if I am still high in the morning woul like to return to 24
u tommorrow night. But what about these morning lows?. Would a bigger
evening snack be the answer?
Regards,
Brian

Default FAO Alan Hardy (diet exercise insulin)

Hi Brian,
Probably a one-off due to God-knows-what - they happen to us poor buggers.
Maybe someone else has a better idea?

What time is your evening snack? I think it should be immediately before
bed, that`s what I do.

As for the one-offs, I just had a whole day in which I tested two hypos (2.2
& 2.6) and one spike (10.3) with absolutely no change in my daily routine. I
have no desire to alter diet, exercise, or insulin dose, and I did not
change anything on that day - just a strange day. A couple of hypos and one
spike will not damage me - but lots of them will.

You are, however, not getting serious damage but slightly too many spikes,
and definitely too many hypos. If we cannot sort out the Lantus/Novorapid,
you could be one of the 7% or thereabouts who simply cannot handle gm
insulin, in which case animal insulin would be required.

Diabetes UK says 5% to 10% of insulin-using diabetics cannot handle gm
insulin.

Lantus is made from porcine insulin by removing one amino acid, so strictly
it is no longer insulin, and since the chemical structure has been altered,
yes, it is gm.
I think if the moved snack does not work, there is one last try - moving
your Lantus to first thing in the morning. But we have not got to that yet.

I would like more suggestions from other ppl, as I am sure you would.

Surely to be GM genes from a different species have to be added e.g. the
genes from cold water fish put into tomatoes so they can resist frost.

Default FAO Alan Hardy (bacteria insulin)

Peter C said this...

In the case of Lantus, the genes of E-Coli bacteria are manipulated. In
this case, the bacteria no longer produces the toxin that makes E-Coli
dangerous, but instead the organisms produce an insulin ****ogue.

Novo insulins do the same with brewers yeast. Lilly use E-Coli for the
same reason.

Paracetamol is made that way too!

To be fair it was YOU that started rattling on about removing amino acids in
response to Brian's cri de coeur.

I shall therefore STFU and try not to go off at a tangent

Lantus split to 2x is worth a go at first. Many people can't make it
work on just one dose. Dosing twelve hours apart.

Also very valid concerns.
There are a good number of people out there (me included) who have tried
Glargine and failed with it. It is much less than a perfect product.

Animal derived insulins are another option, but not one which is readily
offered or considered. It's one of my hobby horses too, although I don't
use it at the moment. I would do in a heartbeat if needed.

Ok Peter,
You are quite right, it is a bit of a hobby-horse, though the temporary
serious damage and permanent slight damage it did to me is something I don`t
wish to happen to anybody else. But I do admit, and often have, that gm
insulin is perfectly suited to a large majority of ppl - but not all.

Thanks, FR. Is that a 50/50 split, or 60/40?
