Carol,

From what I understand, the US government has set up
this Daticon System of tracking bankruptcies for tax
purposes. Once you send the form to Daticon - they
will issue you a claim number, without this claim
number you can not make a claim in the Dow Corning
Bankruptcy Settlement. So, sending in this form is
extremely important. August 30, 2004 is the deadline.

If you have a claim number from Daticon, call SF-DCT
and see if they have the same claim number for you.
If they don't, then I would send in a new form to
Daticon (certified mail with return receipt)and send
a copy to SF-DCT. Also, ask SF-DCT and see what they
say. Better safe than sorry.
