Eczema is not a condition "that occurs where you scratch". It is a skin disorder. The lesions are a symptom of inflamed eczema, but they could also be symptomatic of a host of other disorders, namely psoriasis. If you have not had a proper diagnosis from a dermatologist (not a regular doc, they are not trained in skin problems) then you need to do that first. Scratching can cause lesions to appear at the site. The lesions could also be a clue that you are having an allergic reaction and you scratched that, either from contact or ingestion with/of an allergen. My advice is to seek out a good dermatologist and get a proper diagnosis.
