First, you have to make sure the Exchange Administrator has created the
Organizational Forms Library. After that has been done, then it should appear in the list of places to publish. To find information about setting up a new default form, go to http://www.slipstick.com and search on 'default', without the quotes. One of the search results will tell you all you need to know.
