You are right Bob, I just wanted the template so I won't have to measure the
squares. I can never get the right measurements of anything from the screen
ruler line. But maybe even can do it in Excel.

I'm sure a template exists somewhere as we had it at the last job I was at.

I use label formats for things like this. I imagine that one of the shipping
label formats would be the right size.
