I don't think you understand what I mean. Why have the fields
auto-populate when the user is not going to change them. Is it nessesary
for the user to know the city/state/county before submitting? If not, do
what I said, accept the zip from the user and do the insert into your
database with the relevant info from a select on a table with the
zip->city/state/county info. Understand?

Oh, ok. I understand. I wanted them to see the city, state and county pop
up so they could make sure it corresponded to the zip code (a patient would
give them for entry) before submitting form. Hence, if the zip was wrong in
the database table containing zips, states, and counties (or a client gave
an erroneous zip code) the person using the application would know right
away. Hope that makes sense. I do like the idea of not using javascript and
pulling the information for the zip/city/county/state table upon
submitting, but the people entering the information will feel more
comfortable that it's right before going in.
