At a girl's USA Volleyball club tournament, the libero forgot to come
off the court, leaving 7 players on the court. The official never
noticed there were 7 players on the court and play continued till
there was a side-out. The team with 7 players called a time-out, then
re-entered with 6 players. No one noticed the extra player on the
court, with exception to the team with 7 players.

Had the official noticed there were 7 players on the court, what is
the appropriate call/hand signal? Is it out of rotation?

Yes, signaling out of rotation is the correct call.

The penalty is loss of rally.

Only slightly off-topic...at Nationals last month, I saw a women's
*Open* team get whistled for having only 5 players on the court (libero
went out, and their replacement didn't come on). 'Twas quite amusing...
